A 44 year old male with left flank pain and chronic renal failure who had baseline creatinine level of 2.0      1/29/2012

 

A 44 year old male with left flank pain and chronic renal failure who had baseline creatinine level of 2.0 mg/dl presented to outpatient urology clinic with a ultra sonography which revealed multiple bilateral cortical renal cyst s , the greatest  was 8 cm let side upper pole cortical cyst with fine septations. No hematuria was present at  urine analysis .

A contrast enhanced abdominal ct scan was performed which showed a Bosniak ll left renal cyst, the patient was scheduled for laparoscopic cyst ablation .

 

 

After left ureteral stenting   transperitoneal  laparoscopic approach was initiated toward left kidney by Dr Aghamir, Hamidi and Salavati . fist fluid aspiration was sanguineous . what would be your next step ?

Conversion to open was done by subcostal incision , a patch of cyst wall was excised and sent for frozen section along with necrotic debris filling the cyst . what is your next move ?

 

 

Regarding CRF and  non-benign looking cyst with precise demarcation from normal parenchyma , the cyst was enucleated with proper normal looking margin under manual control of renal pedicle and calyceal repair was checked by infusion of dye into the ureteral stent. Frozen section was reported as suspicious to RCC .  Patient was discharged on 4th post operative day .
